Prior to Your Initial Visit

We recommend that you download and complete the Client Information Form. Please bring this with you on the day of your appointment. Click the button below to download this form.





On the Day of Your Initial Appointment

1.   Be sure to bring any forms that you have filled out in advance, including forms from your referring veterinarian. Any diagnostic test results that have not previously been sent/faxed to us should also be brought to your appointment. In addition, it is a good idea to bring any current medications that your pet may be taking. If your referring veterinarian has given you a completed Referral Form, please be sure to bring that with you as well.

2.   Please plan on arriving 20 minutes early for your initial visit in order to complete any additional forms for your pet's medical record.

3.   You may give your pet water, but please do not feed your pet for 12 hours prior to your appointment (except for very young animals, animals with diabetes or when directed otherwise). Continue your pet's normal medication schedule and bring all medication(s) with you.

4.   If you are unable to keep your appointment, please give us a 24-hour notice. We'll be happy to reschedule.
